I gather what the letter needs to say. Bring any requirements from your surgeon or provider, and your consultation or surgery date if you have one.
You get the letter.
Within a week at most. Sooner when a date is close.
What to have ready
- Any letter requirements from your surgeon or the provider requesting it.
- Your consultation date or surgery date, if you have one.
- The name the letter should use.
You decide what to pay.
Letters are on a pay-what-you-can scale from $0 to $50. There is no application and no proof of income. Choose the amount that works for you.
The letter is on practice letterhead with license and NPI information, as surgeons and insurers expect.
